Information required by FACTS to complete the assessment application process:

– Copies of your most recent Federal Tax Returns including all schedules
– Copies of your most recent W-2 forms for both you and your spouse
– Copies of supporting documentation for Unemployment Benefits, Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, and/or TANF
– A letter of appeal to the Tuition Assistance Committee explaining how your financial situation and outlook has been affected by the Covid-19 economic downturn. Please include specifics such as how much you anticipate being able to contribute vs. how much assistance is needed, as well as whether you might have means to pay back some of the assistance during the school year.
